Choosing Transitional Care After A Hospital Stay

Recover In Comfort & Close To Home At Catholic Eldercare’s Transitional Care Unit

After any hospital stay, many of us yearn for our own surroundings. Being able to snuggle in your own bed and be immersed in the familiarity of your own home seems like paradise. As wonderful as that scenario sounds, the transition from hospital to home may be more complicated, especially for seniors. There are many safety factors to contemplate before you can decide if you’re prepared to head home:

Mobility
Obstacles like chairs, furniture and general clutter in walkways can pose a danger as you’re recovering. Will you be able to maneuver around your house without tripping or falling?

Additional supports such as shower rails, handrails, and toilet supports can be very helpful during recovery. Is your house set up with these additional supports?

Medication
Proper organization of medication, especially if there’s more than one, is important in order to avoid accidental overdose. In addition, taking the correct medicine at the correct time can be crucial to pain management as well as your recovery. Is there someone there to help you fill your prescriptions and remind you when to take them?

Nutrition
It can be challenging to get up and prepare a meal for yourself as you’re recovering. Is there someone there to ensure you’re getting proper nutrition and balanced meals?

Monitoring
It’s important that you are monitored for the first few days home. Dangerous symptoms can arise that should be relayed to your health provider. Is there someone there to help you monitor your symptoms as they arise?

Future Appointments
There may be many follow up appointments as you recover. Is there someone to help you remember where you need to be and when? Do you have a way to get to your follow-up appointments?
